Registered Nurse Care Manager (RN)
Primary Focus
- The Registered Nurse shall practice their profession in a safe and efficient manner.
- The Registered Nurse is able to function independently within the work environment, utilizing strong analytical, problem-solving and decision making skills and is able to effectively handle crisis situations and conflict resolution.
- The Registered Nurse is always aware that the client is a customer of (Organization) Home Health Services and as such, will always maintain a professional and courteous relationship.
Responsibilities:
- Understands and practices within the scope of the Regulated Health Professions Act.
- Uses the nursing process to assess the health status and the needs of the client.
- Plans and implements nursing care plans in conjunction with the client and/or family, and other members of the health discipline team.
- Continually assesses the level of care required.
- Acknowledges and implements physicians orders within the guidelines of the agency and the College of Nurses provincial regulations
- Delivers and documents nursing care on the client chart using the nursing process.
- Communicates to other members of the health discipline team the status of new cases admitted to service and any changes in the client, which require addition/deletion of services.
- Provides direction and acts as resource to members of the health discipline team.
- Utilize the principles of adult teaching/learning to promote client independence.
- Provides a supportive emotional environment.
- Is aware of and utilizes available resources to meet the educational needs of the client.
- Maintains professional and organizational standards of personal behaviour and appearance.
- Ensure confidentiality of client information.
- Keeps abreast of current trends in nursing practice and participates in continuing education programs.
- Adheres to the policies of specific government programs as instructed by (Organization) Home Health Services.
- Makes suggestions to improve caseload management keeping in mind client focused, cost effective, time efficient care.
- Follows the guidelines of the Workplace Hazardous Materials Information System (W.H.M.I.S.)
Education/professional development requirements:
- Current RN registration with the College of Nurses
- Nursing Undergraduate Degree an asset
Experience Requirements:
- Visit: Minimum two years acute care or community nursing preferred.
- Shift/Institution: Minimum one year acute care preferred
- Current CPR certificate
- Valid drivers license, car and proof of automobile business liability insurance
Position scope/key dimensions:
- Acts as a resource to the RPN and unregulated health care provider.
- Works effectively and in complete cooperation with members of the multidisciplinary health care team.
- Responsible to the client for quality care delivered in a timely and cost efficient manner.
