Registered Nurse Position Description

Registered Nurse Care Manager (RN)

Primary Focus

Responsibilities:

  1. Understands and practices within the scope of the Regulated Health Professions Act.
  2. Uses the nursing process to assess the health status and the needs of the client.
  3. Plans and implements nursing care plans in conjunction with the client and/or family, and other members of the health discipline team.
  4. Continually assesses the level of care required.
  5. Acknowledges and implements physicians orders within the guidelines of the agency and the College of Nurses provincial regulations
  6. Delivers and documents nursing care on the client chart using the nursing process.
  7. Communicates to other members of the health discipline team the status of new cases admitted to service and any changes in the client, which require addition/deletion of services.
  8. Provides direction and acts as resource to members of the health discipline team.
  9. Utilize the principles of adult teaching/learning to promote client independence.
  10. Provides a supportive emotional environment.
  11. Is aware of and utilizes available resources to meet the educational needs of the client.
  12. Maintains professional and organizational standards of personal behaviour and appearance.
  13. Ensure confidentiality of client information.
  14. Keeps abreast of current trends in nursing practice and participates in continuing education programs.
  15. Adheres to the policies of specific government programs as instructed by (Organization) Home Health Services.
  16. Makes suggestions to improve caseload management keeping in mind client focused, cost effective, time efficient care.
  17. Follows the guidelines of the Workplace Hazardous Materials Information System (W.H.M.I.S.)

Education/professional development requirements:

  1. Current RN registration with the College of Nurses
  2. Nursing Undergraduate Degree an asset

Experience Requirements:

  1. Visit: Minimum two years acute care or community nursing preferred.
  2. Shift/Institution: Minimum one year acute care preferred
  3. Current CPR certificate
  4. Valid drivers license, car and proof of automobile business liability insurance
