SUPPORTING RESIDENTS AS THEY AGE IN PLACE
When a prospect is wait-listed, you run the risk of loosing them to a competing community. ComForcareSM can help by providing in-home care for the prospect until a room becomes available. TAP is designed to provide support for both wait-listed prospects and residents needing extra assistance during the initial move when transferring to a different room within your facility

TAP Program

How the TAP Program Works

  1. Your staff identifies residents or prospects requiring transition services
  2. ComForcare professional conferences with the community manager, the resident or prospect and their family members, and assesses needs
  3. In concert with individualized one-on-one hourly care, the caregiver provides additional services such as:
    • Moving assistance, including careful packing of delicate and sentimental personal items.
    • Transportation services to the post office, Secretary of Province, utility companies and other errands involved with the move
    • Free installation and transfer of personal response system.
