SUPPORTING RESIDENTS AS THEY AGE IN PLACE
When a prospect is wait-listed, you run the risk of loosing them to a
competing community. ComForcareSM
can help by providing in-home care for the
prospect until a room becomes available. TAP is designed to provide support for
both wait-listed prospects and residents needing extra assistance during the
initial move when transferring to a different room within your facility

How the TAP Program Works
- Your staff identifies residents or prospects requiring transition services
- ComForcare professional conferences with the community manager, the resident or prospect and their family members, and assesses needs
- In concert with individualized one-on-one hourly care, the caregiver provides
additional services such as:
- Moving assistance, including careful packing of delicate and sentimental personal items.
- Transportation services to the post office, Secretary of Province, utility companies and other errands involved with the move
