We specialize in providing non-medical home care services designed to complement the independent living experience. Our caregivers are trained to assist with daily tasks and provide companionship, allowing residents to maintain their routines with dignity. Whether a resident needs help with specific tasks or ongoing support throughout the day, we offer flexible, customizable care plans to suit each individual\u2019s needs.<\/p>\n<\/div>\n<\/div><\/div>\n<\/div><\/div>\n<\/section>

\nA few key beneficial elements of the ComForCare On-Site Model are: \n<\/h2>

  • Experience<\/strong>: ComForCare designed the On-Site Model in 2000, which is still in operation today. This model has been replicated in over 70 independent living communities nationally. I have attached an overview of the program we provide in local communities, providing a better sense of the structure and services provided.\u00a0<\/span><\/li>\n
  • Enhanced Care Services<\/strong>: Beyond task-based care, we start every case with an RN assessment (where allowed) that places emphasis on education and support and place a Program Manager into each community who serves as the go-to between residents, families, our caregivers and the Executive Team. We also offer wellness initiatives in all buildings we partner with, such as Matter of Balance classes, wellness presentations, dementia support groups, coordination of various providers needed to support residents beyond the scope of non-medical care through concierge physician services, PT\/OT, etc.\u00a0<\/span><\/li>\n
  • Integrated Community Partner Approach<\/strong><\/span>: Our goal is to maintain a presence in the community through office occupancy\/hours, servicing residents 24\/7, participating in prospective resident tours, coordinating and participating in joint marketing events, and providing resident activities maintaining a great relationship with the residents above and beyond care delivery.\u00a0<\/span><\/li>\n
  • Improve Population Health<\/strong>: We have a fall risk management program (Gaitway), dementia training program (DementiaWise), and solutions to improve medication management. We are eager to learn on every level what the needs of residents are and standby ready to assist in addressing any challenges leading to high resident turnover and develop solutions.\u00a0<\/span><\/li>\n
  • Home Office and Corporate HQ Oversight<\/strong>: We can always be better together. I am seeking to work with Sonida HQ to learn how we can be a better care provider and in turn, coach our franchise locations to deliver on a local level. ComForCare wants to learn more about your goals for resident care and my role is to help owners on a local level to operationalize those needs into program development that requires a little creativity and the ability to framework design, implementation and ongoing management. This is my current role and is something that I currently work hand-in-hand with owners on a daily basis.<\/span><\/li>\n<\/ul>\n
