Unfortunately, these transitions don\u2019t always go smoothly and a person can be readmitted to the hospital.<\/p>\n<p><strong>Transitional care services from ComForCare Home Care are available 24 hours a day, seven days a week - including holidays. This includes assistance from the hospital, rehabilitation center or skilled nursing facility along with help getting settled in at home.
